Immigration: More Foreign Nurses Needed?

The U.S. nurse shortage is getting worse, but are more visas the answer—or would improved training capacity, working conditions, and pay do the trick?

Editor's note: This is the fourth article in an occasional series on immigration in a recession.

For more than a decade, the U.S. has faced a shortage of nurses to staff hospitals and nursing homes. While the current recession has encouraged some who had left the profession to return, about 100,000 positions remain unfilled. Experts say that if more is not done to entice people to enter the field—and to expand the U.S.'s nurse-training capacity—that number could triple or quadruple by 2025.

President Barack Obama's goal of expanding health coverage to millions of the uninsured could also face additional hurdles if the supply of nurses can't meet the demand.

Some lawmakers are looking to the immigration pipeline as one means to raise staffing levels. In May, Representative Robert Wexler (D-Fla.) introduced a bill that would allow 20,000 additional nurses to enter the U.S. each year for the next three years as a temporary measure to fill the gap. If the bill doesn't pass on its own, lawmakers may include it in a comprehensive immigration reform package. Obama is slated to meet with congressional leaders on June 25 to discuss reforming U.S. immigration laws.

Hospital administrators such as William R. Moore in El Centro, Calif., a sparsely populated town 100 miles east of San Diego, see the Wexler bill as a potential life raft. Moore is chief human resources director at El Centro Regional Medical Center, a 135-bed public hospital that typically has 30 open positions for registered nurses (RNs). While it's hard to lure nurses from nearby big cities (San Diego is 100 miles west), Moore says he could quickly recruit dozens of eager, qualified nurses from the Philippines if the government allocated more visas. "All we want is temporary relief," says Moore. "Let us get a group of experienced RN hires from the Philippines, and we won't ask for more."

Obama begs to differ

Wexler's bill is opposed by labor unions, whose leaders say it would undermine efforts to produce a steady domestic workforce while sapping other nations' nurses. Obama has also expressed skepticism about the idea that the U.S. needs to import nurses, in particular because the U.S. unemployment rate continues to rise. "The notion that we would have to import nurses makes absolutely no sense," Obama said at a health-care forum in March. "There are a lot of people who would love to be in that helping profession, and yet we just aren't providing the resources to get them trained—that's something we've got to fix." The $787 billion economic stimulus bill included $500 million to address shortages of health workers in the U.S., with about $100 million to promote nursing and increase capacity at U.S. nurse-training schools.

The nursing shortage has a number of causes, including an aging workforce, difficult working conditions coupled with stagnating pay, and a lack of capacity at U.S. nursing schools. Peter I. Buerhaus, professor of nursing at Vanderbilt University Medical Center, says the recession has eased the nurse shortage in some areas of the U.S. as more Americans seek out the field's relative job security. Some hospitals also see less need for staff as more Americans lose health insurance and fewer people spend money on elective surgery and doctor visits. But Buerhaus estimates that by 2025 the nurse deficit will be twice as severe as the last major staffing shortage in the mid-1960s, after Congress passed the Medicare and Medicaid programs.

As openings have become more difficult to fill domestically, more foreign-born nurses have entered the workforce, most commonly through green cards that lead to permanent residency. In 1994, 9% of the total registered nurse workforce was composed of foreign-born RNs; by 2008 that percentage had risen to 16.3%, or about 400,000 RNs, according to Buerhaus' research. Of those 400,000 nurses, about 10% had immigrated to the U.S. within the previous five years. About one-third of the increase in RNs from 2001 to 2008 was composed of foreign-born RNs.

Many U.S. nurses choosing not to work

The trend worries leaders of nurses' unions, who say importing workers can lower incentives to improve working conditions.

Understaffing, mandatory overtime, and physically demanding work, such as lifting and bathing patients, take their toll. And while pay has risen in some regions to attract more nurses, in recent years it has flattened at the national level. That's why up to 500,000 registered nurses are choosing not to practice their profession—fully one-fifth of the current RN workforce of 2.5 million. Union leaders say the down economy is a chance to bring these nurses back into the field. "If unemployment is spiking, why do we need to bring in nurses from another country?" asks Ann Converso, president of United American Nurses, which represents 50,000 RNs. "We believe thousands and thousands of RNs would rejoin the profession if conditions improved." Converso says she doesn't oppose all overseas recruitment, but that lawmakers' focus should be on improving staffing ratios in hospitals to improve working conditions. "We have to again allow nurses to do what they do best: care for human beings," she says.

Mick Whitley, managing director of London-based global health-care staffing firm HCL International, says there's no need for alarm about foreign nurses. He points out that since 2006 it has become increasingly difficult for foreign-born nurses to obtain green cards to work in the U.S.; an applicant backlog has built up as annual quotas have been reached. "While patients in U.S. hospitals wait and suffer from a lack of sufficient care, experienced and caring internationally trained nurses who want to come here to help are also waiting [for a green card> for as long as seven years," says Whitley, a former nurse in the U.K. and Australia. "It's great that President Obama has committed more money to expanding health care, but the nurses that will be necessary to staff such expansions are nowhere to be found—at least not here, not yet."

Moore of El Centro Regional Medical Center says his hospital has been waiting for two years for 20 Philippine nurses he recruited to obtain visas. He says in the meantime he's unable to find talent in the area. "We're in the poorest and least literate county in California, right in the middle of the desert," says Moore. "We're not a destination for [American> nurses." Moore has had success hiring Philippine nurses, many of whom choose to stay and settle in El Centro. To them the U.S. "is the land of milk and honey, and the streets are paved in gold," says Moore. "They're not so particular."

Moore denies he wants to hire foreign-born talent to hold down wages. "We pay [a nurse> fresh out of school $28 an hour and $35-$40 with experience," he says.

One point everyone seems to agree on is that the U.S. needs more capacity to train nurses. Since 2002, enrollments at nursing schools have increased so much that up to 50,000 qualified applicants are turned away each year from training programs. The main problem is a lack of teaching staff at these schools. Dan Stultz, president of the Texas Hospital Assn., which represents more than 500 Texas hospitals, helped form the Texas Nursing Workforce Shortage Coalition to push for funding from the state legislature to boost capacity at Texas nursing schools. Stultz says the state has about 22,000 nurse vacancies now, and that the number could rise to 70,000 by 2020. Meanwhile, for the last five years, 8,000 to 12,000 nursing-student applicants have been denied places at training programs for lack of space. "We have qualified people that get accepted and can't attend," says Stultz. "We don't need more immigration; we need to increase capacity and grow our own workforce."

ProEnglish hails court decision as victory for English language learners


ARLINGTON, Va.-"We are thrilled by today's Supreme Court decision in Horne v. Flores that recognizes the demonstrated effectiveness of structured English immersion (SEI) teaching methods for teaching English language learners (ELL)," says K.C. McAlpin, executive director of ProEnglish. The Arlington, Va.-based organization advocates making English the official language of the United States. It joined three other groups in filing an 'amicus curiae' brief in the case.

"The majority opinion states, 'Research on ELL instruction indicates there is documented, academic support for the view that SEI is significantly more effective than bilingual education. Findings of the Arizona Department of Education in 2004 strongly support this conclusion,' " McAlpin added.

The court ruled that the lower court had failed to adequately consider whether the Nogales school district's implementation of SEI was a "changed circumstance" warranting relief.

McAlpin said SEI has proven its superiority to bilingual education wherever it has been implemented and called bilingual education an abject failure that handicaps ELLs for life. He pointed to new numbers from the Arizona Department of Education released this week estimating that 40,000 or 29 percent of ELLs enrolled in SEI classes passed the English fluency exam and will transition into mainstream classes this year up from only 17,813 students or 12 percent of ELLs that passed the English fluency exam after being enrolled in bilingual education classes in 2006-2007.

"This decision is a victory for English language learners and a major defeat for the bilingual education industry," McAlpin says. "It's time to get rid of bilingual education and give English language learners a real opportunity to learn English and succeed," he adds.

I wanted to make you aware of some recent upgrades to the WeHireAliens.com website.

Part of the value of this incredible site is that each time a citizen makes a report about a company, their evidence is submitted to ICE, the FBI, the Social Security Administration and in many cases a state taxing authority--all in one click of the proverbial button!

This does create a challenge, though, because our contact list of law enforcement and agency contacts (which are vary by state) gets out of date with the changes in leadership, reorganizations, etc. We have recently made several changes to our contact list that should reduce the number of emails that don't find the intended recipient. E-Verify is one of the most effective and accurate programs the federal government has ever introduced. As of January, 2009, over 100,000 employers were enrolled in E-Verify, and 15 states had mandated some form of employment-eligibility verification according to the U.S. Citizenship and Immigration Service (USCIS). [SIZE=+0]Remove the incentives that draw illegal aliens looking for work to Ohio, and the non-criminal element will either self-deport or never come here in the first place. [/SIZE]

Watching and reading the mass media, one might conclude that the United States of America is exemplified primarily by New York, Washington, Los Angeles, San Francisco and maybe a few other major cities. But Americans live out their day-to-day adventures in another 3,100 counties sprawled across this continent.
On a frequent basis, we will be introducing new maps that provide additional ways to gauge the quality of life for those who live within a county or state. Because of limitations of available data, we will sometimes not be able to provide comparisons for the smallest counties, but we will usually be able to provide insight for where the vast majority of Americans live.

TODAY, WE FEATURE POPULATION GROWTH SINCE 1980
Rampant, biggest-ever U.S. population growth is one of our gravest concerns at NumbersUSA. One of the most important reasons we advocate reduced immigration is because federal immigration policies are the cause of most U.S. population growth.
Our population maps today will help you see at a glance where immigration is driving the most population growth and radical change across the country. Nearly every county that is colored for high growth either (a) has had a lot of immigration, or (b) has had a lot of migration of Americans fleeing other parts of the state or country that have themselves had very high immigration.

One thing I asked of my tech team was to create impregnated smart cursors. I like to get to information quickly. So my tech team has impregnated the cursor on the maps with an incredible array of statistics and rankings. All you have to do is roll your cursor over a part of the map and the information about that county, state or metro will pop up in a balloon for your view. Then just click to get a close-up or to fill the text box on the left with the information so you can print it.

No. 1: Who were the 7 Senators who switched and brought us victory on E-Verify in the Senate today?

Attempts to attach mandatory E-Verify to the Stimulus Bill in March failed when 50 Senators voted to kill the amendment by tabling it. But today, 7 of those 50 came over to our side. Click to see which NumbersUSA activists from what states we should be thanking.

The vote today put the Senate on record as saying that returning military veterans and other unemployed Americans -- not illegal aliens -- should have government-paid jobs.

No. 2: View the full roll-call vote on Sen. Sessions' amendment to mandate E-Verify for all federal contractors.

How did your 2 Senators vote?

This is the toughest most sweeping of Sessions' amendments, all of which previously failed.

This one:
  • authorizes E-Verify PERMANENTLY
  • mandates that any business getting a federal contract must run all new hires through E-Verify
  • mandates that every existing employee who workS on the government contracts must be run through E-Verify
Thus, for the first time, E-Verify could be used to root out illegal aliens who were previously hired. The existing employee part only applies to that part of a company actually working on the government contract. Still, wouldn't it be nice to know that no illegal aliens are being paid with taxpayer money?

No. 3: Which 6 Democrats defied Party leadership to vote for E-Verify in March and did it again today?

Hint: 5 are from states that partly or wholly are the Great Plains. Here they are.

Included is my salute to Senate Republicans who unanimously went against Big Business and the U.S. Chamber of Commerce. How about that? I'm sure many didn't want to vote with us, but voter pressure in their states has pushed them into becoming champions for American workers.

By the way, was that your state with the Senator who backslid, having voted for E-Verify in March but turning against it today?

No. 4: More phone calls needed Thursday for 2 more amendments.

Our NumbersUSA legislative team is updating by the hour the progress of voting on DHS appropriations amendments.

As I write this, we are still waiting for votes on an amendment by Sen. Grassley (R-Iowa) and Sen. Vitter (R-La.).

Grassley's amendment would allow every employer in America to voluntarily run all their current employees through E-Verify to make sure that none are illegal foreign workers. Right now, it is illegal for an employer to do that. Can you believe it?

Vitter's amendment would re-instate a Bush Administration rule that businesses get notified of all their employees whose Social Security numbers don't match up with SSA records. This would be another way of driving illegal aliens out of their jobs.

It appears that Senate Majority Leader Reid (D-Nev.) is using parliamentary moves to keep those two amendments from getting votes, once he lost on the Sessions and DeMint amendments. He probably will do what he has done often in the past: Call a cloture vote to end debate before the immigration amendments are brought to the floor.

Your phone calls may be the only things that can block this maneuver and force a vote.

The most important Senators to call are those who voted YES to kill the Sessions E-Verify amendment.

Especially if the Democrats' phones are ringing off the hooks Thursday morning, they may tell Sen. Reid that he needs to allow an honest vote on our amendments.

No. 5: Fence vote was an eye-opener.

View the roll call vote of how each Senator stood on finishing the 700-mile fence on the Mexican border today.

7 Republicans voted AGAINST the fence amendment (along with 37 Democrats). The Republicans were:

Voinovich (Ohio)

21 Democrats voted FOR it (along with 33 Republicans). The Democrats were:

Klobuchar (Minn.)
Landrieu (La.)
Lincoln (Ark.)

Can you find a pattern among those Democrats? And notice that in Indiana and Florida, the Democrat supported the fence while the Republican didn't.

And what is going on in Oregon and California? When have we had the support of both Senators in each of those states?
