Bernie Mac had sarcoidosis, but what is it?

By Judith Graham | Tribune reporter
August 9, 2008

Comedian and actor Bernie Mac (Bob Fila, Chicago Tribune / December 6, 2000)

Many news reports about comedian Bernie Mac's death today in Chicago mention that Mac had suffered in the past from a condition called sarcoidosis. The term launched many readers on Internet searches.

Sarcoidosis—what's that?

It's an immune system disorder that affects tens of thousands of Americans. Symptoms include a persistent cough, shortness of breath, fatigue, night sweats, weight loss, small red bumps on the face, arms or buttocks, red, watery eyes, and arthritis in the ankles, elbows, wrists and hands, according to the Mayo Clinic. Most cases are mild, but severe cases can cause scarring in the lungs, a complication that occurs in 20 to 25 percent of patients.

What causes it?

Typically, our bodies fight perceived threats with an inflammatory response. With sarcoidosis, this response becomes excessive and ends up producing small clumps of cells that can cluster throughout the body. Large clusters can interfere with the functioning of organs, most commonly the lungs (more than 90 percent of cases), the eyes and the skin. In addition to lung scarring, it can cause serious eye conditions such as cataracts and glaucoma, or aching and swelling in the legs accompanied by arthritis.

Who gets it?

This is primary an illness of adults in the 20- to 40-year-old range. African-Americans are more prone to the condition than whites (the incidence rate among blacks is 35.5 cases per 100,000; among whites it's 10.9 per 100,000). African-American women are twice as likely as African-American men to be struck with the illness.

Why do people get it?

It's not clear what causes sarcoidosis, though experts believe environmental contaminants can help trigger a genetic susceptibility. Studies have found a link between this condition and irritants such as tree pollen, insecticides and moldy environments.

Can it be cured?

Not currently. The most common treatment is Prednisone, a steroid that can have serious side effects. Occasionally, physicians prescribe drugs known as immune system suppressants (such as Plaquenil and Methotrexate). While sarcoidosis can be severe, less than 5 percent of patients die of it. In the vast majority of cases, symptoms are mild and disappear over time. Mac had the condition until it went into remission in 2005. About one-third of patients have an "unrelenting," progressive form of this illness that typically leads to organ impairment.

Actor, comedian and exasperated dad Mac dies at 50

Bernie Mac blended style, authority and a touch of self-aware bluster to make audiences laugh as well as connect with him. For Mac, who died Saturday at age 50, it was a winning mix, delivering him from a poor childhood to stardom as a standup comedian, in films including the casino heist caper "Ocean's Eleven" and his acclaimed sitcom "The Bernie Mac Show."
Though his comedy drew on tough experiences as a black man, he had mainstream appeal — befitting inspiration he found in a wide range of humorists: Harpo Marx as well as Moms Mabley; squeaky-clean Red Skelton, but also the raw Redd Foxx.
Mac died Saturday morning from complications due to pneumonia in a Chicago area hospital, his publicist, Danica Smith, said in a statement from Los Angeles. She said no other details were available.

Friends, Colleagues Pay Tribute to Bernie Mac.

Hollywood woke up this morning to the news that actor-comedian Bernie Mac has passed away due to complications from pneumonia. He was 50.
As word spreads, tributes are pouring in from the funnyman's friends and colleagues.

Brad Pitt: "I lament the loss of a ferociously funny and hardcore family man. My thoughts are with [his wife] Rhonda and their family. Bernie Mac, you are already missed."

George Clooney: "The world just got a little less funny. He will be dearly missed."

Chris Rock: "Bernie Mac was one of the best and funniest comedians to ever live, but that was the second best thing he did. Bernie was one of the greatest friends a person could have. Losing him is like losing 12 people because he absolutely filled up any room he was in. I'm gonna miss the Mac Man."

Cedric the Entertainer: "It's hard to put into words just how I feel and what a painful loss this is. Bernie was a brother, a friend and one of the comic masters of our time. Sharing the marquee with him during the phenomenon of the Kings of Comedy tour bonded us like family, and created a unique moment in comic history marking some of the most meaningful, memorable and fun times of our lives. His comedic approach was his own brand and will definitely stand the test of time. The level of his talent always inspired me and other comedians to 'bring their A-game.' I promise you that you never wanted to be the guy who had to follow Bernie's set! As a husband and father, he was THE MAN and my thoughts and prayers are with his family. He will truly be missed, but so well remembered."

Don Cheadle: "This is a very sad day for many of us who knew and loved Bernie. He brought so much joy to so many. He will be missed, but heaven just got funnier."

Kelly Preston and John Travolta: "We are heartbroken. He will be deeply missed. He was a wonderful, kind and gentle man."

Ocean's Eleven costar Carl Reiner: “It’s a tremendous loss because of his age and the fact that he was such a vital, original human being. When I use the word 'original' I really mean it. He was like no other person I knew. He lived his life to the fullest, even when we were on the set of Ocean's. He had his own little apartment and he cooked and invited people to lunch every day and he had food that was for everybody. He made very exotic things. His conversations were always different than any conversations I had with anyone else. They were very family-oriented; he talked about his wife and children with such love and it’s very hard to believe that he’s not with us anymore.”

Actress/comedienne Niecy Nash, who played Mac's little sister on the hit television series The Bernie Mac Show and acted opposite him in the 2005 film Guess Who, said this about her friend and mentor: "His passing is such a major loss to the acting and comedy communities. Bernie Mac was the personification of the word 'real.' He kept it real. That kind of genuine spirit that he carried all the time cannot be easily duplicated, but I will do my very best to try."

Fox Broadcasting Company and 20th Century Fox Television: "Bernie Mac was a gifted talent whose comedy came from an authentic and highly personal place. He was a tremendous live performer and a wonderful actor. Fox was proud to be the home of The Bernie Mac Show, and all of us at Fox and 20th Century Fox Television extend our deepest sympathies to his wife, Rhonda, and daughter, JeNiece."

Jenifer Lewis: ”Bernie’s style of comedy was bold, courageous and revolutionary—I never knew anyone who loved to be funny as much as Bernie. He will most definitely be very missed.”

Samuel L. Jackson, Mac's costar in the November 2008 flick Soul Men: "It goes without saying that Bernie was one of the preeminent comedians of our generation. He was also an attentive husband, a great father and loving grandfather. I feel blessed to have shared years of friendship with Bernie Mac and I'm honored to have finally costarred with him in what I consider to be his finest cinematic acting achievement. My sincere prayer is that his family will be comforted by the warmth of love from all of us who knew and respected this man."

Bernie Mac's Death: What Happened?
By Hilary Shenfeld and Ivory Jeff Clinton

Bernie Mac’s wife and daughter were with him until the very end, says the late comic's sister-in-law in a personal and touching interview with PEOPLE that took place Saturday.

Speaking of a heartbreaking moment between her younger sister, Rhonda, and the 50-Year-old actor-comedian – who succumbed to complications from pneumonia in a Chicago hospital at 2 a.m. Saturday – Mary Ann Grossett says that the night before Mac died, "He struggled for his life. He couldn't breathe.

"He opened his eyes on his own and looked at Rhonda. She called his name, and he opened his eyes and nodded to her. She smiled at him and told him, 'Don't leave me … 'I'm waiting for you to come back.' He shrugged his shoulders, and she said that's when she knew he was tired. He signaled to her that his body was tired."

Rhonda, Mac’s wife since 1977, and their 30-year-old daughter, Je'Niece, 30, were with him when he died. "[The doctors] were working on him," says Grossett. "They tried to resuscitate him two times. One time he came back for about an hour. Then he went into cardiac arrest the second time."

Prior to that, the couple had last communicated on July 31. "He told his wife [non-verbally] that he could breathe on his own, and he wanted the ventilator out. He motioned that he wanted it out," says Grossett.

Lung Disease Contributed
The sister-in-law says Mac's inflammatory lung disease contributed to his death. "He had sarcoidosis, but it was in remission," she says. "But because he had it, his immune system was compromised. He had an infection ... He was on a new medication that suppresses the immune system, and that's where the pneumonia came from."

She reveals that Mac – having trouble breathing and running a fever – was actually hospitalized on July 24, eight days before the date given for his admittance. The actor was diagnosed with pneumonia and immediately placed on a ventilator.

"He was critically ill when he was in the hospital," says Gossett. "He was in intensive care the whole time."

She adds that doctors kept the star sedated, although he was conscious at times and he contracted a second strain of pneumonia while in the hospital.

His Wife's Reaction
Of her widowed sibling (like Mac, Rhonda is 50), Grossett says, "She’s devastated. However, she's at peace about his transition because of her faith in God. Her faith is what is sustaining her."

She says of the couple's enduring union, "They had 30 years of marriage. That's unprecedented in today's time, particularly for celebrities. That brings joy to her. They loved each other and respected each other on a daily basis. She was by his side to the very end."

Recalling the couple's early years, when they were still in their teens, Grossett says, "When they started dating, he said, 'Girl, you better come on board this train, because I'm going to be rich.' And her response was, 'Okay,' and they were married. That's how he charmed her, with his sense of humor."

She adds, "When they got married they were kids. He was 20 and she was 19. They both grew up together, and they both matured in this marriage.

'She's Going to Miss Him Dearly'
"What she will treasure the most is the fact that she was his wife for 30 years – and not only was she his wife, but she was his best friend. She's going to miss him dearly."

Mac’s funeral is scheduled for Friday, Aug. 15, at an undisclosed location. The family requests that donations be made to the Bernie Mac Foundation for Sarcoidosis, 40 E. Ninth St., Suite 601, Chicago IL 60605.
