Although he wasn't ever an elite golfer, Ian Baker-Finch is another guy that went from winning a major to being unable to break 80.

Shaun Alexander.

Went from league MVP in 2005 with over 1,800 yards and 27 TDs to crap almost overnight.

The average life of a running back in the league is short. Although it seems like a steep fall, it wouldn't rank as a high one on the list. Plus he did have a bad injury which also played a role.

wtf happened to Steve Francis?

That's a good one, but I'll say this. The guy should never have been a starter for the West in any allstar game. I think he was voted to start 3 times, but the only reason he achieved that vote was because he was Yao Ming's teammate and all the people in China who voted for Yao also voted for Francis.
