Do you mean a "serious disease"
Like "I hope you get cancer is serious" BUT "I hope you get an std" is not

OR

do you mean as in a serious vs joking manner

Serious....."Put a shotgun in your mouth and pull"
Joking...."Go jump off a tall bridge."
