I forgot to answer your post regarding Costa Rican books...

While I agree with you that there are more sports books located there that go belly up, it's directly related to the fact that they have many many more sports book located there than anywhere else in the world...

So simple probability and statistics would make Costa Rica the favorite to have the highest number of successul sports books and at the same time, the most failures, too...
